PLEASE FOLLOW THESE INSTRUCTIONS FOR RECORD RELEASE:RECORDS WILL BE RELEASED ONLY TO PATIENTS/PARENTS/GUARDIANS WITH VALID ID(DUE TO SIGNIFICANT ELAPSED TIME FROM CLOSURE KIDZ HEALTH CANNOT RESPOND TO URGENT OR EMERGENCY REQUESTS)​​​​​​​

1)Send us a message via WhatsApp to 773-685-3288 and request a Record Release Authorization Form. (WhatsApp is a free and secure downloadable app.)
2) Complete the form, sign it, and send it to us - alongwith a copy of your StateID/Passport -using WhatsApp to 773-685-3288
3) Patients currently over the age of 18 years and emancipated patients must sign the forms on their own behalf.
4) The records will be sent back through WhatsApp in a Zipped file format. Generally you can double click the file and follow instructions for your operating system.
5) It takes 9 - 12 weeks for the record release process to complete, due to limited resources.
6) Once you receive the Zipped Record file folder please download immediately since WhatsApp deletes files after 3-4 weeks
7) If you request the above record release more than once, there will be a $35 charge for each additional release per patient.

FOR IMMUNIZATION RECORDS ONLY: Your new Medical Provider can also access and download the vaccine records from the Illinois ICARE Immunization registry at https://DPH.Partner.Illinois.Gov
